This website is dedicated to inspiring, sharing and learning about how, together, we can enhance collaboration and communication among the health team. The health team is inclusive of all members: patients/clients/community members, family members, health professionals, non-health professionals, health team administrators, etc.

Collaborate for Health promotes interprofessional collaborative practice (IPCP) as the gold standard for health teams. IPCP involves a dynamic and coordinated approach to healthcare delivery, bringing together the health team to work collaboratively, drawing on their unique skills and perspectives, to enhance patient care, improve outcomes, and streamline healthcare processes. Communication, mutual respect, and shared decision-making are central tenets, creating an environment where each team member contributes their expertise to create a comprehensive care plan that supports the patient in achieving their health goals. As advocates for patient-centered care, interprofessional collaborative practice aims to break down silos, optimize resource utilization, and deliver more effective, holistic care.
