Interprofessional collaboration can be improved by knowing what health providers or professionals should be included on the team based on their training. As a patient or client, it is helpful to know who you might consider inviting to your health team based on your needs. As a provider, being aware of team members that can contribute to the patient's health will be helpful. The patient/client and their caregiver(s) are always members of the team, so you will not see them repeated under each medical condition. Our hope is that every patient/client has a primary care provider (PCP), so you will not see the PCP repeated under each medical condition as well.
